Monstrous Regiment: Why Females?

My question is why did Prachett name the book "Monstrous Regiment"? Was it only because it included a vampire, igor, and ogre? Or was it because it consisted of FEMALES as well as a vampire, igor, and ogre? Is it really difficult for the American culture to accept women in authoritative positions? Also, what do you think was the significance of the generals in the Borogravian military being women?

The title comes from an essay by John Knox called "The First Blast of the Trumpet Against the Monstrous Regiment of Women", an early invective against women ruling men. You can read about it at http://en.wikipedia.org/wiki/The_First_Blast_of_the_Trumpet_Against_the_Monstrous_Regiment_of_Women.

There is also a tradition in the UK of naming articles of clothing after successful military men (The Wellington Boot, the Cardigan). A blouse is a woman's shirt, and also used as a put-down for a weak or useless man.
